How Does Dry Needling Work?

The Science

Dry needling involves the insertion of thin, monofilament needles into myofascial trigger points—hyperirritable spots within a taut band of skeletal muscle. These trigger points are often associated with pain, decreased range of motion, and muscle dysfunction. Needle insertion stimulates the localized tissue environment, causing a mechanical disruption of the tight band, which can help release tension and restore muscle elasticity. This mechanical input also promotes local blood flow and the release of biochemicals such as endorphins, which facilitate pain relief and tissue repair. Simply put, the needle helps "reset" the muscle by reducing tightness, improving circulation, and alleviating discomfort.

From a central nervous system (CNS) perspective, dry needling engages sensory afferent pathways, sending signals to the spinal cord and brain. This input can modulate pain perception through the gate control theory, where non-painful stimuli inhibit pain signals, and by stimulating descending inhibitory pathways, which reduce overall pain sensitivity. Additionally, needling may decrease hyperactivity in motor neurons, allowing muscles to relax. In layman’s terms, the nervous system "rewires" itself to reduce pain and restore normal muscle activity, creating a lasting impact on both the targeted area and the brain’s response to discomfort.
