Physical Therapy Evaluation

Service Description

Physical Therapy Evaluation includes 1 hour of one on one with a Doctor of Physical Therapy. Includes patient/family interview, establishing patient goals, vital signs check, pain assessment, strength and range of motion screen, baseline functional status, ability to perform activities of daily living, standardized functional tests. In short, this will answer the following questions: What is wrong? How can we help? What can you currently do? What do you want to be able to do?

To Cancel or Reschedule. Please contact us 24 hours in advance. Any cancellation with less than 24 hour notice is subject to a cancellation fee of $30. Thank you for understanding.
